The review phase of a performance management period is a time for supervisor and employee to discuss the work accomplished, and any challenges that came up.
This may be a daily discussion, a weekly discussion, or the traditional yearly discussion. The important thing is that it takes place.
The supervisor is a coach rather than a judge. Regardless of the type of form used, employee should leave the discussion table with a plan for future growth and accomplishment, rather than just revisiting past performance.
